BridgeCare physicians and their team of Nurse Practitioners and Physician assistants provide quality one-on-one attention to elderly patients in their own home. BridgeCare physicians serve seniors in assisted living, independent living, retirement communities, senior apartments, and private homes.
BridgeCare physicians will make monthly visits to provide:
- Monthly Assessments and check ups
- Wellness Visits – annual wellness visit evaluations for Medicare Patients
- Monitor and Manage chronic diseases
- Manage complex care
- Coordinate care with Patient’s PCP and other specialist
- Arrange referrals for specialists
- Arrange community referrals based on patient’s need
- Provides Sick or Urgent visits
- Medication Management including Prescription Refills
- Pain Management
- Podiatrist visit and services
- Preventative care
- Diabetes education
- Flu shot/ Immunization
