On-Calls Physicians

Our on-call physicians are available 24/7 to answer your questions and guide you in an emergency. Designed to provide peace of mind for the patient and their family member knowing that their physicians will be available when they need them most. BridgeCare operators are available around the clock to provide real time connections to medically directed clinical support.

BridgeCare physicians will coordinate care with the appropriate department in the case of hospitalization. BridgeCare‘s staff will coordinate the care including medication reconciliation after post-hospital discharge to mitigate any medication related issues.

BridgeCare physicians also provide transition care to patients post-hospitalization to prevent or reduce chances for re-hospitalization.
