What services does BridgeCare provide?

BridgeCare offers comprehensive house-call medical care throughout the greater Chicagoland area. Our services include routine primary care visits, chronic disease management, medication management, preventive care, annual wellness visits, transitional care, sick visits, diagnostic testing coordination, and more.
We serve adults, seniors, and patients who have difficulty traveling to a clinic.

Who is eligible for home visits?

Patients who have difficulty leaving home without assistance—or whose health conditions make travel challenging—qualify for medical house calls.
We serve:

  • Patients with chronic medical conditions
  • Individuals recently discharged from the hospital or rehab
  • Medicare and private-pay patients
Do you accept insurance?

Yes. BridgeCare accepts Medicare, most Medicare Advantage plans, and select commercial insurance plans.
We also offer private-pay options.
Our billing team can verify your insurance before your first visit.

How do home visits work?

A BridgeCare physician or advanced practice provider (APRN/PA) comes directly to your home at the scheduled appointment time. During the visit we can:

  • Perform a full examination
  • Review medications
  • Manage chronic conditions
  • Address new symptoms
  • Coordinate tests, labs, imaging, and specialty referrals
  • Provide preventive and wellness care

Most services performed in a clinic can be provided in the home.

Can you order labs, imaging, or medications?

Yes. Our providers can:

  • Order labs (bloodwork, urine tests, cultures)
  • Order X-rays, ultrasounds, and EKGs
  • Prescribe medications
  • Coordinate with pharmacies, specialists, hospitals, and home health agencies.

Results are sent directly to BridgeCare and reviewed promptly.

How often do providers visit?

Visit frequency depends on medical need. Many patients are seen monthly or quarterly, while others require more frequent follow-up.
Your provider will establish a schedule tailored to your health needs.

Do you offer urgent or same-week visits?

Yes. Whenever possible, BridgeCare offers same-week and urgent appointments to address sudden illness, infections, medication issues, hospital discharges, or other concerns.

Can family members be involved in the visit?

Absolutely. We encourage family involvement and can communicate with caregivers, family members, and designated power of attorney—with patient permission.

How do I become a BridgeCare patient?

What areas do you serve?

BridgeCare serves all of Chicagoland from Wisconsin to Indiana. Some of our patients reside in Des Plaines, Lombard, Schaumburg, Hoffman Estates, Elgin, Naperville, Aurora, Oak Brook, Downers Grove, Chicago, and surrounding Chicagoland communities.

Are telehealth visits available?

Yes. For appropriate conditions, BridgeCare offers telehealth video visits, especially for medication follow-ups, care plan updates, and coordination with families.

Is BridgeCare HIPAA-compliant?

Yes. BridgeCare follows all HIPAA regulations and uses secure, encrypted medical record and communication systems to protect your health information.
